Nursing’s records to the control of hydric balance

The data registered in the hydric balance should be trustworthy, avoiding false evaluation and, consequently unnecessary interventions. Objective: The objective was to analyze the nursing records accomplished in the hospitalized patients' hydric balance. Methods: The work is documental, developed in the unit of intensive therapy of a public hospital of Fortaleza-Ceará. 16 handbooks of patients interned for at least three days were analyzed, totaling 48 printed papers. A structured form was used, containing aspects related to the volumes of administered and eliminated liquids. The data were exposed in tables. Results: As for the results, all of the handbooks possessed records of hydric balance. However, only 28 records (58,3%) were complete; 40 (83,3%) forms were readable; 27 (56,2%) records didn't have erasure. Regarding the administration of medicines, 45,8% of the forms didn't consist the records of the liquids administered orally. The record regarding the administration for road endovenosa is the most frequent (93,7%). The numeric records of the amount of the diurese were present in 36 forms. Conclusion: Therefore, the results appeared that the nursing annotations in relation to the hydric balance need to be reviewed and reorganized, to get better the elaboration of the plan of cares, which should be systematized, individualized and humanized.
